If yes, you must agree to the following or your child will not be able to ride the bus to MPBC:

"My permission is granted for any Mount Pleasant Baptist Church staff member or sponsor in charge of the trip to obtain medical attention in case of sickness or injury for child list above.

"I/We, the undersigned, do hereby release, remise and forever discharge all sponsors and Mount Pleasant Baptist Church, Colonial Heights, Virginia, from any and all claims, demands, actions, or cause of action, past, present, or future arising out of any damage or injury while participating in this event. I also agree to pick up my student or reimburse MPBC for the cost of sending my student home in the event that he/she commits any infraction of the rules, puts their fellow participants in danger, or exhibits a disruptive attitude."

My child is allowed to attend the Good News Club at Mount Pleasant Baptist Church after school every Tuesday that the club is scheduled to meet. Club will run from Oct 1 through Nov 19, 2024 (will not be meeting Nov 5). I understand it is my responsibility to pick up my child at 5:00PM and failure to do so will jeopardize my child’s continued participation.

Child Evangelism Fellowship® may, from time to time, document the activities of the ministry with photos or videos.

I hereby assign and grant to Child Evangelism Fellowship Inc., its subsidiaries and successors, and assign the unqualified right to the ownership, use and proceeds of all photographs or video of me or my minor child, without reservation or limitation, including use of photographs or video of me or my minor child for, but not limited to, advertising, educational and promotional purposes.
